If IL-1 is prevented from binding to its receptor, the inflammatory response decreases. The first IL-1Ra that has been approved by the Food and Drug Administration for use in people with moderate to severe rheumatoid arthritis who haven't responded adequately to conventional DMARD therapy is anakinra (Kineret). It may be used alone or in combination with methotrexate. Anakinra is given as a daily self-administered injection under the skin. Some potential side effects include injection site reactions, decreased white blood cell counts, headache and an increase in upper respiratory infections. There may be a slightly higher rate of respiratory infections in people who have asthma or chronic obstructive pulmonary disease. If you have an active infection, don't use anakinra.




* Abatacept (Orencia). Abatacept, a type of costimulation modulator approved in late 2005, reduces the inflammation and joint damage caused by rheumatoid arthritis by inactivating T cells — a type of white blood cell. People who haven't been helped by TNF blockers might consider abatacept, which is administered monthly through a vein in your arm (intravenously). Side effects may include headache, nausea and mild infections, such as upper respiratory tract infections. Serious infections, such as pneumonia, can occur.




* Rituximab (Rituxan). Rituximab reduces the number of B cells in your body. B cells are involved in inflammation. Though originally approved for use in people with non-Hodgkin's lymphoma, rituximab was approved for rheumatoid arthritis in early 2006. People who haven't found relief using TNF blockers might consider using rituximab, which is usually given along with methotrexate. Rituximab is administered as an infusion into a vein in your arm. Side effects include flu-like signs and symptoms, such as fever, chills and nausea. Some people experience extreme reactions to the infusion, such as difficulty breathing and heart problems.




* Antidepressant drugs. Some people with arthritis also experience symptoms of depression. The most common antidepressants used for arthritis pain and nonrestorative sleep are amitriptyline, nortriptyline (Aventyl, Pamelor) and trazodone (Desyrel).




Surgical or other procedures





Although a combination of medication and self-care is the first course of action for rheumatoid arthritis, other methods are available for severe cases:




* Prosorba column. This blood-filtering technique removes certain antibodies that contribute to pain and inflammation in your joints and muscles and is usually performed once a week for 12 weeks as an outpatient procedure. Some of the side effects include fatigue and a brief increase in joint pain and swelling for the first few days after the treatment. The Prosorba column treatment isn't recommended if you're taking angiotensin-converting enzyme (ACE) inhibitors or if you have heart problems, high blood pressure or blood-clotting problems.




* Joint replacement surgery. For many people with rheumatoid arthritis, medicines and therapies can't prevent joint destruction. When joints are severely damaged, joint replacement surgery can often help restore joint function, reduce pain or correct a deformity. You may need to have an entire joint replaced with a metal or plastic prosthesis. Surgery may also involve tightening tendons that are too loose, loosening tendons that are too tight, fusing bones to reduce pain or removing part of a diseased bone to improve mobility. Your doctor may also remove the inflamed joint lining (synovectomy).

Self-care





Treating rheumatoid arthritis typically involves using a combination of medical treatments and self-care strategies. The following self-care procedures are important elements for managing the disease:




* Exercise regularly. Different types of exercise achieve different goals. Check with your doctor or physical therapist first and then begin a regular exercise program for your specific needs. If you can walk, walking is a good starter exercise. If you can't walk, try a stationary bicycle with little or no resistance or do hand or arm exercises. A chair exercise program may be helpful. Aquatic exercise is another option, and many health clubs with pools offer such classes.




It's good to move each joint in its full range of motion every day. As you move, maintain a slow, steady rhythm. Don't jerk or bounce. Also, remember to breathe. Holding your breath can temporarily deprive your muscles of oxygen and tire them. It's also important to maintain good posture while you exercise. Avoid exercising tender, injured or severely inflamed joints. If you feel new joint pain, stop. New pain that lasts more than two hours after you exercise probably means you've overdone it. If pain persists for more than a few days, call your doctor.




* Control your weight. Excess weight puts added stress on joints in your back, hips, knees and feet — the places where arthritis pain is commonly felt. Excess weight can also make joint surgery more difficult and risky.




* Eat a healthy diet. A healthy diet emphasizing fruit, vegetables and whole grains can help you control your weight and maintain your overall health, allowing you to deal better with your arthritis. However, there's no special diet that can be used to treat arthritis. It hasn't been proved that eating any particular food will make your joint pain or inflammation better or worse.




* Apply heat. Heat will help ease your pain, relax tense, painful muscles and increase the regional flow of blood. One of the easiest and most effective ways to apply heat is to take a hot shower or bath for 15 minutes. Other options include using a hot pack, an electric heat pad set on its lowest setting or a radiant heat lamp with a 250-watt reflector heat bulb to warm specific muscles and joints. If your skin has poor sensation or if you have poor circulation, don't use heat treatment.




* Apply cold for occasional flare-ups. Cold may dull the sensation of pain. Cold also has a numbing effect and decreases muscle spasms. Don't use cold treatments if you have poor circulation or numbness. Techniques may include using cold packs, soaking the affected joints in cold water and ice massage.




* Practice relaxation techniques. Hypnosis, guided imagery, deep breathing and muscle relaxation can all be used to control pain.




* Take your medications as recommended. By taking medications regularly instead of waiting for pain to build, you will lessen the overall intensity of your discomfort.




Coping skills





The degree to which rheumatoid arthritis affects your daily activities depends in part on how well you cope with the disease. Physical and occupational therapists can help you devise strategies to cope with specific limitations you may experience as the result of weakness or pain. Here are some general suggestions to help you cope:




* Keep a positive attitude. With your doctor, make a plan for managing your arthritis. This will help you feel in charge of your disease. Studies show that people who take control of their treatment and actively manage their arthritis experience less pain and make fewer visits to the doctor.




* Use assistive devices. A painful knee may need a brace for support. You might also want to use a cane to take some of the stress off the joint as you walk. Use the cane in the hand opposite the affected joint. If your hands are affected, various helpful tools and gadgets are available to help you maintain an active lifestyle. * Know your limits. Rest when you're tired. Arthritis can make you prone to fatigue and muscle weakness. A rest or short nap that doesn't interfere with nighttime sleep may help.




* Avoid grasping actions that strain your finger joints. Instead of using a clutch purse, for example, select one with a shoulder strap. Use hot water to loosen a jar lid and pressure from your palm to open it, or use a jar opener. Don't twist or use your joints forcefully.




* Spread the weight of an object over several joints. For instance, use both hands to lift a heavy pan.




* Take a break. Periodically relax and stretch.




* Maintain good posture. Poor posture causes uneven weight distribution and may strain ligaments and muscles. The easiest way to improve your posture is by walking. Some people find that swimming also helps improve their posture.

On the other hand, people afflicted with rheumatoid arthritis generally have pain caused by inflammation and often benefit from aspirin or other nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen (Motrin or Advil). Ibuprofen combines anti-inflammatory with pain relief, but does typically cause some stomach discomfort if not taken with food and have been linked to ulcers with long-term use.




Glucosamine and Chondroitin





Glucosamine and Chondroitin are components of natural joint cartilage. Studies indicate that glucosamine and chondroitin can help alleviate joint pain related to osteoarthritis and improves general joint function including greater joint flexibility. It appears that together they help offset joint space narrowing, the loss of cartilage within the joint, seen with osteoarthritis by stimulating production of new cartilage. Both glucosamine and chondroitin are synthesized by the body and are naturally found in cartilage. Researchers theorize that glucosamine assists in the creation of new cartilage, while chondroitin slows cartilage destruction. Some studies indicate that glucosamine may help as much as nonsteroidal anti-inflammatory drugs (NSAIDs) such as aspirin, ibuprofen, naproxen, and tolmetin in relieving symptoms of osteoarthritis, particularly in the knee, with fewer side effects.




Prescription Products

Prescription Nonsteroidal Anti-Inflammatory Drugs (NSAIDs)



This class of drugs includes aspirin and ibuprofen that are used to reduce pain and inflammation, as noted above, and may be used for both short-term and long-term relief in people with osteoarthritis and, more commonly, rheumatoid arthritis. NSAIDs also include Vioxx (rofecoxib), Celebrex (celecoxib), and Bextra (valdecoxib), all of which are classified as COX-2 inhibitors. COX-2 inhibitors work by blocking an enzyme known to cause an inflammatory response. COX-2 inhibitors had been approved for short-term use in the treatment of pain, and for longer-term use to treat the signs and symptoms of osteoarthritis and rheumatoid arthritis.




Biological Response Modifiers





These drugs include Enbrel and Remicade and are used for the treatment of rheumatoid arthritis through reducing the inflammation in the joints. It is theorized that they block the reaction of a substance called tumor necrosis factor, an immune system protein involved in immune system response.




Disease-Modifying Antirheumatic Drugs (DMARDs)





These drugs include Arava, methotrexate, hydroxychloroquine, penicillamine, and gold injections. Typically, these are drugs used to treat people with rheumatoid arthritis who have not responded to NSAIDs or are no longer taking NSAIDs because of the FDA recall of Vioxx, Celebrex, and Bextra (COX-2 inhibitors). It is theorized that these drugs influence and correct abnormalities of the immune system responsible for a disease like rheumatoid arthritis and other rheumatic diseases. Due to the likelihood of adverse side effects, treatment with these medications requires careful monitoring by the physician.




Corticosteroids





Corticosteroids can be taken by mouth or given by injection directly into the affected joint. Prednisone is the corticosteroid most often given by mouth to reduce the inflammation of rheumatoid arthritis. In both rheumatoid arthritis and osteoarthritis, the doctor also may inject a corticosteroid into the affected joint to stop pain. These hormones are very effective in treating arthritis but cause many side effects, including damage to the cartilage. In fact, frequent injections may cause damage to the cartilage, and as such, they should only be done once or twice a year.Hyaluronic Acid Products.




These drugs include Hyalgan and Synvisc. It is theorized that these drugs mimic a naturally occurring body substance that lubricates the knee joint and permits flexible joint movement without pain.




Natural Methods





Heat and Cold





Since heat and/or cold is not recommended to alleviate symptoms associated with all types of arthritis, the decision whether to use it or not should be discussed with your doctor or physical therapist. If appropriate for use on your arthritis pain, it must be determined which kind of temperature treatment should be used. Moist heat, such as a warm bath or shower, or dry heat, such as a heating pad, placed on affected joint for about 15 minutes may relieve the pain. An ice pack wrapped in a towel and placed on the sore area for about 15 minutes may help to reduce swelling and stop the pain. If you have poor circulation, do not use cold packs.




Joint Protection





A splint or brace can be used to allow joints to rest and keep them from being used, which can exacerbate the condition and may lead to additional injury. As with many other treatments, your physician or physical therapist can make recommendations and possibly provide you with the brace.




Massage



This method is associated with temporarily relieving one of the major symptoms associated with arthritis, joint pain, rather than treating the underlying cause, loss of cartilage. A massage therapist will lightly stroke and/or knead the painful muscle, which increases blood flow to the stressed area. Exercise/Weight Reduction



Stretching exercises, swimming, walking, low-impact aerobic exercise, and range-of-motion exercises may reduce joint pain and stiffness while increasing joint mobility. In addition to alleviating some symptoms, the weight loss associated with an exercise program is beneficial in relieving the stress of extra weight on weight-bearing joints, especially the hips and knees. Studies have shown that overweight women who lost an average of 11 pounds substantially reduced the development of osteoarthritis in their knees. In addition, if osteoarthritis has already affected one knee, weight reduction will reduce the chance of it occurring in the other knee. A physical therapist or gym trainer can help plan an exercise program that will give you the most benefit with the least stress on the arthritis-stressed joints.




Medical-Related Methods





Transcutaneous Electrical Nerve Stimulation (TENS)





Transcutaneous electrical nerve stimulation (TENS) is a small device that is placed over the area afflicted with arthritis and directs mild electric pulses to nerve endings in and around the arthritic joint. TENS seems to work by blocking pain messages to the brain and by modifying the body's perception of pain. It may relieve some joint pain associated with arthritis but doesn't seem to offset the inflammation that is associated with arthritis.




Surgery





In advanced cases of patients with arthritis, surgery may be necessary. The surgeon may perform an operation to remove the synovium (synovectomy), realign the joint (osteotomy), or in extreme cases, replace the damaged joint with an artificial one (arthroplasty). The skin has there layers: The outer layer called epidermis consists of epithelial cells. Under this lies the dermis consisting of connective tissue. At the bottom there is a layer called the hypodermis consisting mostly of fat cells.




The skin has narrow pores extending from the surface down to the top of the sub-dermis, called hair follicles. A hair extend from a growth zone in the bottom of each hair follicle and out at the skin surface. Around the hair in the dermis, there are small glands called sebaceous glands. These glands produce a fatty substance, sebum, which empty through the follicle opening and lubricates the hair and the skin.







THE ACNE PROCESS





Acne starts by the glands in the hair follicles increasing their sebum production. This will in the beginning only cause greasy skin. Eventually the sebum in the entrance of the follicles mixes with dead epithelial cells. This mix reacts chemically to forms hard props, comedones that close the pore entrances. According to the color of the comedones, they are called black heads or white heads.




Then the closed hair follicle gets filled with sebum and swell. The swollen follicle then gets infected by bacteria. The bacteria and the accumulated sebum cause the follicle and the surrounding skin to get inflamed, forming a pimple. At last the inflamed follicle burst and empties its content. Eventually the affected area heals.



 Sometimes the inflammation reaches deep into the skin surrounding the follicle and destroys skin tissue. This will cause more or less distinct scars that may reside permanently. Typically an affected person will have follicles in all these stages of the process at any given time.







THE CAUSES OF ACNE





The increased hormone production in the puberty stimulates to increased sebum production. The male hormone, testosterone, seem to contribute most. Also girls begin to produce more testosterone in the puberty. Also in other periods, altered hormone production may give acne, for example by women in the menopause.




Since not all teens get acne in any degree, the increased hormone level cannot be the whole cause of the increased sebum production. The follicles of affected persons must for some reason react stronger upon the higher hormone level.




Dietary factors, like eating too much fat, too much sugar or the wrong kind of fat may aggravate acne by some individuals. Lack of vitamins, minerals and essential fatty acids may also contribute to development of acne.

Very frequent washing and use of strong soaps aggravate acne, and does not alleviate the condition as many think. Rubbing, scrubbing or squeezing the skin may also aggravate acne.

- Environmental or domestic pollution.

- Humid environments.

- Some antidepressants.

- Testosterone and anabolic steroids used for treatment or by body-building.

- Exposing the skin for chlorine or other halogens, or medicines containing halogens.

- Women may get aggravated acne 2-7 days before menstruation.

Sometimes Acne is so severe that special medical treatment is required, and sometimes the causes of acne necessitate medical treatment. But you can do much yourself also. Here are listed things you can do yourself to prevent or treat acne:




1. Some advices about diet that may help prevent and cure acne:




- Do not consume a great amount of fat.

- The fat you add to the food should be natural oils. Olive oil is ideal, but use other types of oils too, like walnut oil, sunflower oil and soy oil. However, do not use only soy oil as many tend to do. Using only soy oil will give you too much of some fatty acids and too less of others.

- Eat much fish, seafood and not so much meat.
